Explain principles of care for clients with oncological disorders.

Scenario

Anna is a 45-year-old female that presented to her physician’s office for her annual check-up. Anna has a history of diabetes, obesity, and noncompliance with diet and medications to control her diabetes. She a single mother of three teenagers and smokes regularly. During the history review, Anna shares with you that she has not been feeling like herself for the past six months, she has been unusually tired and thought that she felt a lump in her right breast during a self-breast exam around that same time. She stated, “I am very busy with my children; I haven’t had time to get it checked out.” She has recently been experiencing right nipple pain. Anna has a positive family history of breast cancer; both her mother and grandmother have been treated for breast cancer. Anna has never had a mammogram. During the breast examination, the practitioner palpated a lump in Anna’s right breast. No discharge from the nipple was observed. Anna’s right breast was tender upon palpation. No abnormalities were found in the left breast. Based on the physical findings Anna will undergo a diagnostic mammogram.

Instructions

In a 2-page paper, describe the care that Anna would require and address the questions below.

  1. What risk factors does Anna have that could predispose her to the development of cancer?
  2. What signs and symptoms could indicate that Anna has developed cancer?
  3. Based on Anna’s risk factors and presenting problems, identify three care strategies that you would use to provide quality care to Anna. Provide rationale to explain why you chose these strategies.

PART 2; Multidimensional Care of the Cancer Patients

Discussion

Cancer is one of the leading causes of death in both men and women. Caring for the cancer patient and family requires a multidimensional approach.

What does it mean to provide a multidimensional approach? What are some examples of how the care team can meet the patient and the family’s needs? Who are the members of the care team and how are they involved in providing multidimensional care

PART 3; WRITTEN ASSIGHNMENT-ACID-BASE IMBALANCE

Competency

Describe strategies for safe, effective multidimensional nursing care for clients with acid-base imbalances.

Scenario

Tony is a 56-year-old, Hispanic male that presented to the Emergency Room with complaints of shortness of breath, which he has been experiencing for the past two days. He states “I haven’t felt good for about a week, but couldn’t afford to miss work.” He complains of a cough, fever, and feeling exhausted. Past medical history includes asthma, chronic obstructive pulmonary disease and diabetes. Upon physical examination, you notice that Tony is struggling to breathe, his respiratory rate is 36 breaths per minute and labored, heart rate 115 beats per minute, blood pressure 90/40 mm Hg, and his pulse oximetry is 84% on room air. You notify the MD. He orders oxygen at 4 L via NC and an arterial blood gas.

Tony’s ABG results:

pH 7.28
PaCO2 – 55 mm Hg
PaO2 – 70 mm Hg
HCO3 – 30 mEq/L

You notify the provider, and he orders oxygen at 4 L via nasal canula NC. Sarah’s chest x-ray reveals bilateral pneumonia. Her arterial blood gas result are below:

pH

7.30

PaCO2

58 mm Hg

PaO2

78 mm Hg

HCO3

26 mEq/L

Sarah is admitted to a general medical floor. You are the nurse assigned to Sarah.

  1. What potential problems can occur based on the above findings?
  2. How would you provide multidimensional care for Sarah?
  3. Describe the roles of other departments in Sarah’s treatment plan

PART 5. ASSIGNMENT- DIGESTIVE POSTER

Competency

Select appropriate nursing interventions for clients with lower gastrointestinal disorders.

Scenario

Patient and family education is important in increasing adherence. To provide education to patient and families, you are going to create a poster about a gastrointestinal disorder that will be displayed in a physician’s office.

Instructions

Please be sure to include the following in your poster design.

  • Overview of the disorder
  • Causes of the disorder
  • Diagnostic tests to diagnosis the disorder
  • Assessment findings
  • Multidimensional nursing care interventions for the disorder

PART 6; ASSIGNMENT -ENDOCRINE DISORDERS CUIDE

Competency

Evaluate responses to nursing interventions for clients with endocrine disorders.

Scenario

You are working as a nurse supervisor. You are finding a lot of new nurses are unfamiliar with certain endocrine disorders. You have decided to put together a guide for these nurses to educate them on different endocrine diseases and provide them with ways to provide quality multidimensional care.

Instructions

Compare and contrast two endocrine disorders within the guide. Include the following information:

  • Identify and compare the causes and diagnostic tests.
  • Identify and compare the signs and symptoms of the disorder.
  • Describe the nurses’ role in caring for a patient that suffers from this disorder to include the multidimensional aspects of nursing care.
  • Identify how you will evaluate responses to the interventions taken for each disorder

part 7.discussion- Influence of Multidimensional Care Elements on Disorders

Disorders of the endocrine system affect many individuals. Providing multidimensional patient care can be challenging for patients experiencing these disorders. Ensuring the plan of care meets the patient and family needs is important in order to increase adherence to proper medical treatment following discharge.

What does it mean to provide a multidimensional approach? Provide at least three examples of how the care team can meet the patient and the family’s needs? List at least three care team members and how are they involved in providing multidimensional care?

Introduction:

As a medical professor, my responsibility is to create college assignments and answers for medical college students. These assignments are designed to enhance the students’ knowledge and skills required to provide quality patient care. In this assignment, I will answer questions related to nursing care of a cancer patient, multidimensional care of the cancer patient, acid-base imbalance, care for SARAH, digestive poster, endocrine disorders guide, and the influence of multidimensional care elements on disorders.

Part 1:

Competence: Explain principles of care for clients with oncological disorders.

Anna’s risk factors that could predispose her to the development of cancer include her family history of breast cancer, diabetes, obesity, and smoking history. The signs and symptoms that could indicate that Anna has developed cancer include the lump in her right breast, tenderness upon palpation, and right nipple pain.

Three care strategies that can be used to provide quality care to Anna include (1) promoting adherence to a healthy lifestyle and medication management, (2) educating Anna on the importance of screening and early detection of breast cancer, and (3) providing emotional support and counseling throughout the diagnosis and treatment process. These strategies were chosen to promote early detection, reduce the risk of recurrence, and provide support during the diagnosis and treatment process.

Part 2:

Multidimensional Care of the Cancer Patients

A multidimensional approach refers to providing care for the patient and their family that addresses their physical, emotional, and social needs. Examples of how the care team can meet the patient and the family’s needs include providing emotional support, education on the disease process and treatment options, pain management, nutrition support, and coordinating care among various specialists.

The members of the care team include the primary care physician, oncologist, nurse, social worker, pharmacist, and nutritionist. They are involved in providing multidimensional care by assessing the patients’ needs, developing a comprehensive care plan, and collaborating to address the patient’s physical, emotional, and social needs.

Part 3:

Competency: Describe strategies for safe, effective multidimensional nursing care for clients with acid-base imbalances.

Tony’s acid-base imbalance is respiratory acidosis. Possible causes of this imbalance include chronic obstructive pulmonary disease and asthma exacerbation. The signs and symptoms that Tony is exhibiting as a result of the acid-base imbalance include shortness of breath, labored respiratory rate, tachycardia, and decreased pulse oximetry.

The multidimensional care strategies that are appropriate for the care of Tony include (1) oxygen therapy to improve oxygenation, (2) respiratory support to improve ventilation, (3) bronchodilators and corticosteroids to reduce airway inflammation, and (4) education on maintaining proper nutrition and hydration status. These strategies were chosen to improve Tony’s respiratory function, reduce complications, and promote recovery.

Part 4:

Discussion – Multidimensional Care of SARAS

Based on the findings, potential problems that can occur with Sarah may include worsening respiratory distress, respiratory failure, sepsis, and acute kidney injury. Multidimensional care for Sarah should involve monitoring vital signs, oxygenation, and hydration status. Providing oxygen therapy, antibiotics, and bronchodilators will also be important.

The roles of other departments in Sarah’s treatment plan include the respiratory therapist, physical therapist, occupational therapist, social worker, and dietician. The respiratory therapist will provide expertise in oxygen administration and respiratory therapy, while physical and occupational therapists will provide support to assist with mobility, etc.

Part 5:

Competency: Select appropriate nursing interventions for clients with lower gastrointestinal disorders.

The poster created should include an overview of the lower gastrointestinal disorder, causes, diagnostic tests, common assessment findings, and multidimensional nursing care interventions for the disorder, e.g., inflammatory bowel disease. Nursing interventions include encouraging fluid and electrolyte balance, managing pain, administering medications as prescribed, providing dietary consultation, and providing education on self-management.

Part 6:

Competency: Evaluate responses to nursing interventions for clients with endocrine disorders.

Comparing and contrasting hypothyroidism and hyperthyroidism under the endocrine disorders guide as examples, nurses can provide a multidimensional approach to care by conducting a thorough patient assessment, developing a comprehensive care plan, educating the patient on their condition, and encouraging adherence to medications. Interventions that are appropriate for hypothyroidism can include thyroid hormone replacement, synthetic TSH, etc. While for hyperthyroidism, interventions can include anti-thyroid medication, radioactive iodine therapy, etc.

Part 7:

Discussion – Influence of Multidimensional Care Elements on Disorders

Providing a multidimensional approach to patient care means addressing the patient’s physical, emotional, and social needs during treatment. Examples of how the care team can meet the patient and the family’s needs include providing emotional support, education on the disease process and treatment options, pain management, and coordinating care among various specialists. Care team members include nursing personnel, doctors, social workers, and therapists.

Part 8:
