Introduction:

Medical research plays an important role in advancing our understanding of medicine and human health. As a medical professor, it is my responsibility to ensure that students are exposed to a wide range of research topics and approaches to conducting research. This assignment focuses on the topic of medical research ethics, diversity in clinical trials and research, and the role of cultural competence in healthcare research.

Part 1:

1. Was this study unethical in 1932?
The study conducted in 1932 regarding syphilis was unethical as it involved deceiving a vulnerable population and harming them instead of providing medical treatment. The study was started with good intentions, but it turned out to be harmful to the participants. As the study progressed, it became even more unethical, and the researchers did not do enough to protect the participants. The study’s unethical nature is widely acknowledged today, and it has led to the development of several regulations and guidelines to safeguard human subjects in research studies.

2. How could the study have been done differently?
The researchers could have conducted the study differently by providing the necessary medical treatment to the participants instead of deceiving them. They could have informed the participants about the study’s objectives and obtained their consent. The researchers could have protected the participants from harm by administering the necessary medical treatment and monitoring their health status throughout the study.

3. What was Nurse Rivers’ role in the project?
Nurse Rivers was responsible for recruiting and monitoring the participants in the study. Her role was to ensure that the participants received the necessary medical treatment and that their health status was monitored throughout the study. She was also responsible for collecting data and reporting it to the study’s principal investigator.

4. Was Nurse Rivers negligent or did she commit malpractice?
Nurse Rivers did not commit malpractice or negligence in the study. Her role was to recruit and monitor the participants, which she did to the best of her ability. She was not the principal investigator of the study and did not have the authority to make decisions about the study’s design or objectives.

5. What was the impact of recruiting at churches and schools on test subjects?
Recruiting at churches and schools was a way to target African American males, who were seen as being more susceptible to syphilis. The impact of recruiting at these locations was that it created an environment where vulnerable individuals could be taken advantage of. However, it also gave individuals access to medical treatment that they may not have otherwise received.

6. Think of a research question or study pertinent to a specific race, ethnicity, or culture.
How does diet impact the risk of developing diabetes in the Hispanic population?

7. Imagine you are the nurse in a modern-day, ethical, TSS-like study.
In conducting a modern-day TSS-like study, cultural competence is important, and the five elements of cultural competence (cultural awareness, cultural knowledge, cultural skill, cultural encounter, and cultural desire) will be used. For the selected population, cultural awareness and knowledge will be used to understand their beliefs, values, behaviors, and needs. Cultural skill and encounter will be used to communicate with the participants and provide culturally sensitive care. Lastly, cultural desire will be used to foster a positive and respectful attitude toward the participants.

8. Which modern-day, government agencies could be involved in a health-related research study?
The National Institutes of Health (NIH) is a government agency that funds health-related research studies. The Food and Drug Administration (FDA) is another government agency that could be involved in a health-related research study. The Centers for Disease Control and Prevention (CDC) is another agency that could be involved in a health-related research study.

Part 2:

1. Hypothesize one reason why a lack of diversity has been a persistent issue in clinical trials and research.
A lack of diversity in clinical trials and research can be due to several factors, including the underrepresentation of certain racial and ethnic groups, the lack of access to healthcare, the mistrust of the medical system, and cultural barriers.

2. What is one potential solution for your hypothesized reason?
One potential solution for addressing the underrepresentation of certain racial and ethnic groups in clinical trials and research is to increase community engagement and involvement in the research process. Researchers can work with community organizations and leaders to build trust and establish partnerships. They can also provide culturally sensitive healthcare services and education to the community to empower individuals to participate in clinical trials and research studies. Additionally, researchers can create targeted recruitment strategies that reflect the community’s demographics and cultural norms.
