Create a PowerPoint presentation that addresses each of the following points/questions. Be sure to completely answer all the questions for each bullet point. Part 1: Choose and explain one Evidence Based Practice Model from Chapter 13 of the textbook (Melnyk and Fineout-Overholt, 2015). Then, related to your research topic from Mod 1, describe in detail how you would utilize the model to implement an evidence-based practice change in your personal clinical practice environment. For this part, I have chosen the ACE Star Model of Knowledge Transformation. This model has five steps which are Ask, Critically Appraise, Synthesize, Translate, and Implement. The Ask step involves formulating a clinical question based on a clinical problem or knowledge gap. The Critically Appraise step involves finding and appraising the evidence related to the clinical question. The Synthesize step involves summarizing the evidence and combining it with clinical expertise, patient preferences, and values to form a strategy for implementation. The Translate step involves planning and implementing the evidence-based practice change. Finally, the Implement step is evaluating the effectiveness of the implementation plan.
Part 1: Applying the ACE Star Model of Knowledge Transformation
For my research topic in Module 1, I chose to focus on the prevalence and risk factors of pressure ulcer development in patients with limited mobility. In my clinical practice environment, I would utilize the ACE Star model to implement evidence-based practice changes as follows:
-Ask: I would ask a clinical question such as “What is the most effective intervention for preventing pressure ulcer development in patients with limited mobility”?
-Critically Appraise: I would search for and critically appraise the available evidence related to the clinical question, such as systematic reviews or meta-analyses.
-Synthesize: Based on the evidence, I would formulate an implementation strategy, which could include interventions such as regular repositioning, skin inspection, nutritional support, and education for patients and caregivers.
-Translate: I would plan and implement the evidence-based practice change based on the chosen interventions, involving a multidisciplinary team, and adapting the plan to the specific patient population and setting.
-Implement: I would evaluate the effectiveness of the implementation plan by measuring the incidence and severity of pressure ulcers following implementation and modifying the plan as needed.
Part 2: Change Model/Theory choice
For this part, I have chosen the Kotter and Cohen’s Change Model. This model has eight steps that involve creating a climate for change, engaging and enabling the organization, creating and communicating the vision, empowering others to act on the vision, creating short term wins, consolidating gains and producing more change, anchoring new approaches in the organization’s culture, and reviewing and institutionalizing the change.
Part 2: Applying Kotter and Cohen’s Change Model
For my research topic, I would utilize Kotter and Cohen’s Change Model to implement evidence-based practice changes in my clinical practice environment as follows:
-Creating a climate for change: I would communicate the importance of the issue, involving leadership and other stakeholders, and creating a sense of urgency.
-Engaging and enabling the organization: I would create a team of staff members who share the same passion for the issue, building relationships with stakeholders, and identifying and addressing barriers to change.
-Creating and communicating the vision: I would create a shared vision of the desired outcome, emphasizing the benefits for the patients, staff, and organization, and communicating the vision regularly and clearly.
-Empowering others to act on the vision: I would provide staff with the necessary skills, resources, and support to implement the evidence-based practices, encouraging collaboration and fostering a sense of ownership.
-Creating short term wins: I would celebrate small successes along the way, highlighting the benefits of the change and providing motivation for further progress.
-Consolidating gains and producing more change: I would build on the successes achieved, continuing to address barriers and improving the implementation process.
-Anchoring new approaches in the organization’s culture: I would ensure that the changes become part of the organization’s culture, by institutionalizing them into policies and procedures, and encouraging staff to embrace them.
